Application Process

  • Apply for Medicaid if you don’t have it (online at kynect.gov or in person at your local Medicaid office)

  • Apply for HCB Waiver (you must have Medicaid before applying; Heartsong will assist you)

  • A Nurse assigned by the Department for Medicaid Services will assess you to determine that you meet nursing facility level of care requirements

  • Heartsong will help you connect with a Case Manager of your choice to assist you with the waiver application and your care plan

  • Heartsong will coordinate with you and your Case Manager to establish care services
